To open a commercial (Net 30) account with Ctek completion of the following forms is required. If your firewall permits downloading of zip files the complete package is available with one download. Otherwise each individual form may be obtained in PDF format.
To complete a purchase using a credit card (MC/Visa/Discover only) download the following credit card form and return it to Ctek via FAX.
